New Patients

Prior to coming to our office, we request that you call to speak to our office assistants. They are your first point of contact with our office. New patients are requested to provide some general information about their reason for seeking services at the Cadenza Center. This enables us to match clients with the best provider for their specific needs. Our office staff also can assist you with insurance verification, advise you of your copay or session fees, as well as give you detailed directions to our office.  

All new patients should complete these forms prior to the first office visit when at all possible. If you choose to complete these forms at our office, we request that you arrive approximately 15 minutes prior to your session in order to allow sufficient time to complete them. This allows you to maximize your time with your therapist during your first session.

Our Fees

Our fees vary by therapist. Our licensed therapists' fees range from $125 to $250 per 45-50 minute appointment. providers are 'in network' with some insurance plans. However, if you are not a member with one of the plans listed below, we are able to accept some insurance plans with out-of-network privileges or PPO's. Your fees will vary depending upon your benefits. Our staff will verify your benefits and discuss your payment obligations prior to your first session.

Our music therapists fees range from $45 per session (for groups) to $100 per individual session in our office. In some cases, music therapy services may be conducted in your home for an additional fee. While music therapy is not a service specifically covered by mental health/insurance, it may be possible for you to obtain music therapy services with either Dr. Michelle Reitman and Dr. Alan Reitman at our Hollywood location. Note, we can not accept straight Medicaid patients. However, persons seeking music therapy services whose child is enrolled in the Medicare Consumer Directed Care Plus (CDC+) program are often able to choose Cadenza Music Therapy to provide behavior therapy, speech therapy, play therapy, psychotherapy, or family therapy.

Under the Agency for Health Care Administration (Medicaid), the CDC+ program is administered by the Florida Department of Elder Affairs. The partner agencies in the program are the Florida Departments of Health, Children and Families, and the Agency for Persons with Disabilities. In Florida, the CDC+ Program developed from a pilot project for approximately 1.100 consumers. Since then the program has expanded. Currently CDC+ is only enrolling participants of the control group from the original pilot project, but general information about the program can be found on this Web site or by calling 1-866-232-3733 .

Please note that payment is due at the time services are rendered, unless other arrangements have been made in advance. We accept checks, money orders and cash. We do not currently accept credit or debit cards as payment.
