Prior to coming to our office, we request that you call to speak to our office assistants. They are your first point of contact with our office. New patients are requested to provide some general information about their reason for seeking services at the Cadenza Center. This enables us to match clients with the best provider for their specific needs. Our office staff also can assist you with insurance verification, advise you of your copay or session fees, as well as give you detailed directions to our office. All new patients should complete these forms prior to the first office visit when at all possible. If you choose to complete these forms at our office, we request that you arrive approximately 15 minutes prior to your session in order to allow sufficient time to complete them. This allows you to maximize your time with your therapist during your first session.

Minors

Child Intake Packet – Filling out this packet will provide us with the information we need to begin working to better your child.

Adults

Adult Intake Packet – This packet will provide us with the information we need to work with adolescents and adults

General

Autism Clinic Intake Packet – This packet provides us with all the information we need to address your childs autism

Art Therapy Release Form – This packet allows us to use the art your child make to evaluate them

Teletherapy Consent – This packet discusses our terms for teletherpay

I wanted to know if art therapy applies to adults as well ? or only children?
